New Atlantic HPV CenterIntegration is the key when it comes to making sure patients get the best health care. To ensure that patients have optimal health results when they leave the hospital, Atlantic Health System offers the Transitions of Care program to patients. We focus on helping patients understand what the next steps are in terms of follow-up with their doctor, getting their medications, taking care of themselves at home, just coordinating and navigating the system. New Vitality: Walk with a Health Care Professional
Tour the mall with a physician, who will discuss a health care topic and answer your questions as you walk. A light breakfast will be provided.

Healing Arts for the Caregiver
Music and art workshops offer communal peer support and a creative outlet to help reduce stress.

Bone Density Screenings for Women
Find out if you are at risk for developing osteoporosis with a noninvasive ultrasound that provides immediate results. Counseling and educational materials also are provided. Registration is required.

Smoking Cessation
Receive the support and guidance needed to quit smoking in six weeks. This group is free and open to all adults in the community, but registration and a smoking assessment are required to participate.
